It is true that the rash does not spread. The outbreaks can occur over a period of time due to a number of factors including thickness of skin, amount of urushiol oil, and sensitivity to the oil. I've seen quotes of rashes breaking out over a period of days and sometimes up to 10 days. I suspect that the accepted norm is a couple days (2-3). Here are a few references:
